PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of preventing transmission destination information including a facsimile number, a mail address, etc., from leaking out. SOLUTION: The image forming apparatus includes: a display means 2 of displaying the transmission destination information including the facsimile number, mail address, etc., of a customer; a transmission control means 3 of transmitting an image to a transmission destination displayed on the display means 2; a data read means 5 of reading transmission destination information and authentication information out of a storage medium 4; a storage means 6 of storing and managing authentication information corresponding to the authentication information on the side of the storage medium 4; an authentication means 7 of deciding whether the authentication information of the storage device 4 side read by the data read means 5 matches the authentication information on the side of the storage means 6; and a display control means 8 of displaying the transmission destination information read by the data read means 5 on the display means 2 when it is decided that those pieces of authentication information match each other.
