PROBLEM TO BE SOLVED: To provide a communication terminal device capable of generating address image data in consideration of even additional information to be added to an address name. SOLUTION: An address information table 12a is a database for storing information regarding addresses of facsimile transmission and e-mail transmission. Any one of "flag data" of the additional information table 12b is stored in an "additional information" field of the address information table 12a as additional data. An address image data generation part 63 generates the address image data on the basis of records extracted from the address information table 12a by an extraction part 62. The "additional information" in consideration of the "address name" is added to the address image data. By this, a user can easily generate the address image data in consideration of the "address name" without executing complicated work.
