PROBLEM TO BE SOLVED: To obtain a form for molding a precast plate with which an assembly work of reinforcing rods is easy to perform, concrete does not ooze, and which can be assembled without hindrance despite a slight displacement of the reinforcing rods. SOLUTION: The form is stood around a part on which a bottom plane of the precast plate for a foundation bed is molded. Reinforcing rods holes are provided at the portions where the reinforcing rods of the side frame project. The reinforcing rods holes are closed by closely attaching a closing panel which is provided with tight holes tightening around the projecting reinforcing rods and is divided so as to let through the rods. Further, the closing panel is pressed to the side frame by a pressing means. Thus, the form for molding the precast plate with which an assembly work of reinforcing rods is easy to perform, concrete does not ooze, and which can be assembled without hindrance despite a displacement of the reinforcing rods is obtained.
