PROBLEM TO BE SOLVED: To provide a main spindle head unit for a machine tool, which can prevent the occurrence of displacement and vibration even when executing heavy cutting while being an attachment main spindle head replaced with a tool changer. SOLUTION: The main spindle head unit for a machine tool is composed so as to provide a support pin 19 and a short support pin 21 to a frame 13 of the attachment main spindle head 7, and also, a positioning block 23 and a movable positioning block 25 on the tip face of a main spindle housing 1. A taper shank 15 of the attachment main spindle head 7 is mounted to a tool gripping part 5 of a main spindle 3. The support pin 19 is engaged with the positioning block 23 and the short support pin 21 is engaged with the movable positioning block 25. The unit is also composed so as to prevent a tool change arm from colliding with the positioning block by retreating the movable positioning block 25 with a cylinder 27 and a piston 29 when replacing the attachment main spindle head 7.
