A supplemental sheet (200) has supplemental information fields which are printed, where the sheet is connected to the folded sheet such that the supplemental sheet is folded open between a position, in which the reverse side of the supplemental sheet is uncovered, and another position, in which the front side of the supplemental sheet is uncovered.
