<p><P>PROBLEM TO BE SOLVED: To provide a fastener to be used during endoscopic procedures in which a suture is used in creating a predefined pouch through tissue apposition within the stomach. <P>SOLUTION: The fastener 100 includes a substantially W shape fastener body 102 having first and second anchors 104, 106 to be extended downward from the substantially W shape fastener body 102. The body 102 includes segments which are constituted to be mutually moved and arranged by making four angles. The first and second inner segments are connected so as to form a vertex part for reliably holding the suture 118, i.e., an opening 116. The third and fourth outer segments are respectively extended from the first and second segments so as to support the anchors extended from the third and fourth segments.
