PROBLEM TO BE SOLVED: To prevent influence of interlayer cross-talk in a single-sided two layer information storage medium. SOLUTION: An ending position of a data region of a layer 1 is positioned on the outer peripheral side of a starting position of a data region of a layer 0 by a clearance corresponding to the interlayer cross-talk, a data lead-in region of the layer 0 includes a guard track zone formed by adding clearances to both sides of a test zone in a data lead-out region of the layer 1 and the data lead-out region includes a guard track zone formed by adding clearances to both sides of a test zone and a management zone in the data lead-in region.
