PROBLEM TO BE SOLVED: To allow an information input server to change a storage location of input information in each input item or to perform identification of disclosure/nondisclosure. SOLUTION: This information input server has: input/output item definition information 211 including an input/output item definition related to input of disclosure information and an input/output item definition related to input of nondisclosure information; an input/output item definition acquisition part 110 acquiring the input/output item definition information 211; an input form production part 121 producing an input form of the disclosure information and the nondisclosure information on the basis of the acquired input/output item definition information 211; a reception part 120 receiving data inputted by a terminal device 10 on the basis of each input form, and outputting the disclosure information of the received data to a disclosure information DB 130; a nondisclosure information output part 150 outputting the nondisclosure information of the data received by the reception part 120 to a nondisclosure information DB 220; and an information disclosure part 140 making the disclosure information stored in the disclosure information DB 130 be browsed via a network.
