An apparatus for generating shock wave and a shock wave treatment apparatus are provided to generate shock wave from a tapered cylindrical shock wave source, thereby to enlarge focus area and to lower energy level. The apparatus includes a pulse power generator which generates pulse current, a shock wave generator(200), which generates shock wave using the pulse current, electrically connected to the pulse power generator to apply the pulse current generated from the pulse power generator, and a shock wave concentrator(300) concentrating the shock wave generated by the shock wave generator at a desired focus portion. The shock wave generator has a tapered cylindrical structure. The shock wave treatment apparatus has an ultrasonic image probe(401) for obtaining ultrasonic image.
