A recording medium for managing forms is provided to easily track a problem caused from the form by inserting ID data of a form provider into image contents of the form in case that a customer provides the form as a settlement tool at a shop having a CAT(Credit Authorization Terminal) and/or a POS(Point Of Sale) terminal, which include a function of a form management terminal. A unique information reading function reads unique information from the forms provided from the form provider. An ID information reading function reads ID information from an ID tool of the form provider. A transmitting function links the unique information for the form with the ID data of the form provider, and stores the linked data to a memory(175) or a storage medium or transmits the linked data to a server on the network. The unique information reading function reads the unique information from the form by linking with an OCR(Optical Character Reader) or an MICR(Magnetic Ink Character Reader). |
