An array type print head and an inkjet image forming apparatus with the same are provided to reduce manufacturing costs of the print head by manufacturing the print head with polymer resin. An array type print head includes a first member(131), a second member(140), a third member(150), and at least one head(160). The first member, the second member and the third member are sequentially deposited. The at least one head is mounted on the third member and has a plurality of ink ejection nozzles arranged in a width direction of a paper to form a plurality of nozzle lines. The first member has a plurality of introduction holes for introducing different colors of ink into the second member. The second member includes a plurality of channels for separately accommodating the different colors of ink, and a discharge hole for discharging the ink to the third member. The third member includes a plurality of path grooves, and supply holes formed at the respective path grooves.
