PROBLEM TO BE SOLVED: To provide a waveguide device that can be easily manufactured and that can be coupled with other optical devices such as an optical fiber with a low coupling loss. SOLUTION: The waveguide device is provided at least with a lower clad layer, an optical waveguide core, and an upper clad layer on a substrate, and is characterized in that concave machining is applied to form a concave leading to the center of the core end face of the optical waveguide or, with such end face as the center, leading to the center of the end face including the lower and upper clad layers. Also, the invention includes a waveguide module in which the waveguide device is coupled with an optical fiber. The module is characterized in that a distance between the end face of the waveguide device and the end face of the optical fiber is 3-15μm, and that a ratio (n<SB>1</SB>/n<SB>2</SB>) of the effective refractive index (n<SB>1</SB>) in the optical waveguide core of the waveguide device over the refractive index (n<SB>2</SB>) in the gap between the waveguide device and the optical fiber is≥1.005.
