PROBLEM TO BE SOLVED: To provide a delivery slip with an envelope upper paper piece which does not fall off during delivery routine by designing an adhesive part of a conventional duplication delivery slip whose envelope upper paper piece is peeled off and used as a copy of a delivery addressee, in such a way that the adhesive part is constituted of a strong adhesive part and a feeble adhesive part existing side by side. SOLUTION: This delivery slip with an envelope is used in the way that the envelope upper paper piece of the delivery slip attached to an article to be delivered is peeled off and used as a copy of the delivery addressee or a delivery slip. In addition, not less than one slip is put into the envelope in the way that the former can be drawn out, and the envelope upper paper piece and the envelope lower paper piece can be stuck by the adhesive part comprising at least two different kinds of adhesive structures.
