The reamer (1) has a base (3) with a geometrically determined cut (9) provided in the base and having cutter (7). A guide rail (17) is provided which is inserted with the cutter into the base with their exterior surface standing out over an extended surface (5) of the base. The guide rail made of a harder material as the material of cutter. The exterior surface of the guide rail is a polycrystalline diamond (PKD). The entire cutter is made of tungsten carbide. An independent claim is included for a method of manufacture.
