PROBLEM TO BE SOLVED: To provide a paper patterning sheet for a concrete decorative form, for extremely easily performing its manufacture, by forming an excellent and beautiful continuous recess-projection pattern even on a surface of a long size concrete mold such as a concrete foundation of a house. SOLUTION: This paper patterning sheet 10 for the concrete decorative form stretched over a form body and forming the recess-projection pattern on the surface of the concrete mold, is constituted by applying a recess-projection-shaped pattern having the recess-projection height of 3 mm or more up to the top of a projection part 20 from the bottom of a recessed part 22 by press working to a sheet of using high extension paper 14 having rupture elongation of 10% or more as a base material 15.
