PROBLEM TO BE SOLVED: To provide an index formation sheet allowing anyone to print easily by using a usual printer for a personal computer, etc. SOLUTION: An index sheet has perforation for separation prepared for forming a tongue of a trapezoidal shape in an end section of an arbitrary side other than the binding side of a standard size sheet, i.e., a JIS A size or B size sheet, and a cut opening for hooking the tongue. A desired index is printed by a printer in the index entry section of the tongue. Then, the tongue of a trapezoidal shape is formed by breaking and separating the perforation. The tongue is folded along a folding line inward or outward, and hooked in the cut to form the index sheet. COPYRIGHT: (C)2007,JPO&INPIT
