PROBLEM TO BE SOLVED: To provide a foam sheet for a panel for forming a wall that has a number of dips formed in a front surface thereof when concrete is placed, in order to prevent separation of mortar applied to an upper surface of the concrete, the dips being formed by using shapes of caps made of the plastic foam sheet, wherein the caps are prevented from being caught by the hardened concrete and easily removable from a form without damaging the caps. SOLUTION: The cap made of the plastic foam sheet is molded such that a height H thereof is set in the range of (0.3 to 0.5) D, where D represents a diameter, and preferably in the range of (0.35 to 0.45) D, that a ridge of the cap is rounded, and that a radius R of curvature is set in the range of (0.1 to 0.3) D and preferably in the range of (0.15 to 0.20) D. It is also preferred that the cap is molded like a truncated cone, i. e. a side wall of the cap is inclined inward, and that a tapering angleθthereof is set to 15 degrees or less with respect to a perpendicular line.
