PROBLEM TO BE SOLVED: To provide device and method for forming a reference hole capable of precisely detecting rotation (an angleθ) of a film and surely correcting the rotation. SOLUTION: This device 1 for forming the reference hole is provided with a movable table 5 to receive the film 4 on a surface of a table main body 3 having two opening parts 2, a punching means 9 combined with punch 7 and die 8 to punch the reference hole 6 in the film 4, an irradiation means 10 to irradiate the film 4 with light beam, a visual field demarcating means 11 constituted of an upright cylindrical body, and an image input means 13 capable of inputting the light beam transmitting the film 4 and passing through the visual field demarcating means 11 as an image signal in a computer.
