PROBLEM TO BE SOLVED: To propose an application blank, in which the inconvenience of the conventional envelope-incorporated type application blank is solved and the papers of which are made uniform in size and bound together. SOLUTION: A means, in which a guide/explanatory paper, an application blank and an envelope paper are made of sheets of paper having one and the same size piled up in the order named and binding parts provided at the side edge parts of respective papers are separably bound together so as to constitute an envelope-incorporated type application blank, is adopted. Further, the guide/explanatory paper has an explanatory column, in which a title, various explanation, writing gists or the like are written according to the spirit of the application blank. Further, the application blank consists of required number of sheets of paper pieces and has duplicatable blanks and is made it possible to cut off through a cutting-off perforation. Furthermore, the envelope paper consists of a front paper and a rear paper so as to form an enclosing part by pasting both the papers by means of a U-shaped pasting part and, at the same time, is made separably possible as an envelope through a cutting-off perforation provided along the enclosing part.
