PROBLEM TO BE SOLVED: To enable each guiding pathway to be brought closer to a specified side surface of an ultrasonic probe as much as possible in a puncturing adapter which is attached to the ultrasonic probe. SOLUTION: A guiding mechanism 20 has a base member 24 and a cover member 26. The cover member 26 is positioned and held to the base member 24 by a first engaging structure 30, a second engaging structure 32, and a third engaging structure 34. In the guiding mechanism 20, an opening section and a protuberance which form the first engaging structure 30 are formed between two guiding grooves, i.e., a special overhang structure is not required to be provided on the specified side surface side of the ultrasonic probe 10. Therefore, each guiding pathway can be brought closer to the specified side surface.
