A waterproof form panel and a manufacturing method thereof are provided to prevent deformation of the form panel and bacteria propagation by extending a waterproof film to an edge of plywood and restricting moisture from being infiltrated into the plywood in curing concrete. An adhesive is spread on the surface of plywood(10), and a waterproof film(20) is mounted in the plywood. The adhesive is spread to an edge of the plywood, and extension parts(22a,22b,22c,22d) of the waterproof film are bent and mounted to the edge of the plywood to manufacture a waterproof form panel(100). The waterproof film is manufactured by using a PET(Polyethylene Terephthalate) film or a deposit PET film formed by vacuum-coating aluminum particles in the PET film. The waterproof film is integrally formed by pressing and bonding aluminum foil in the PET film with heat. The edges of the plywood and a rear part of the plywood are surrounded with the extension parts of the waterproof film.
