<P>PROBLEM TO BE SOLVED: To provide an electronic form provision system reducing a load of generation maintenance work of electronic form data registered in a server device providing an electronic form when a use form is modified, and reducing a load of electronic form data generation processing in the server device in time of a request. <P>SOLUTION: In this electronic form provision system, the server device and a client device are connected by a network. The server device transmits electronic form basic shape data, additional data added to an electronic form basic shape, and an electronic form synthesis script generating input field data from the additional data and synthesizing them to the electronic form basic shape data.
