<P>PROBLEM TO BE SOLVED: To provide an electrosurgical instrument capable of minimizing pressure decline inside a body cavity and appropriately eliminating smoke by simple constitution. <P>SOLUTION: In the case of executing aeroperitonia by injecting carbon dioxide into an abdominal cavity 13 by an air sending apparatus 5 and executing the treatment of incision or the like by implanting an electric scalpel 32 in the state of securing an observation visual field by a camera-mounted endoscope 8, when an output switch 38 for performing the instructing operation of high frequency output is turned ON, after the time set beforehand as a condition of starting a smoke eliminating operation by a setting part 51, a control part 24 operates a suction pump 25 and starts the operation of discharging the smoke generated during the treatment of the incision or the like.
