PROBLEM TO BE SOLVED: To provide a name order card which is made solid to have many faces, instead of a flat name order card having only surface and backside. SOLUTION: An index plate is formed into, for example, a pentagonal prism. Indexes are written in stickers 6, and the stickers are stuck to faces of the index plate.
