PROBLEM TO BE SOLVED: To improve a read-out speed of information and also to read the information without lack and loss against a main cause of external disturbance (defect) such as dust, scratch, etc., corresponding to openings. SOLUTION: The information to be recorded is divided into units of the information amount recordable on the openings which form an opening pattern (Step 101) and divided into recording units (Step 102). Next, it is combined with the information of the opening unit (Step 103) and an encoding of an error correcting code or an encoding of the recording is carried out to form an original image of a hologram reproducing image (Step 104), and a phase and amplitude for producing the hologram with respect to the original image are calculated and the hologram calculation is performed for converting the phase and amplitude to the position and size of an unevenness diffracting the waveguiding end in the laminated waveguide hologram memory (Step 105), and finally, the unevenness is recorded on a core layer (boundary plane between the core layer and clad layer) of the laminated waveguide hologram memory (Step 106).
