Provided is a computerized form data creation system capable of creating computerized form data easily. Identification number giving means (13) gives an identification number automatically to the data indicating plural display areas (3) in computerized layout data. Information output means (15) outputs information on the contents which can be displayed together with the identification number in the plural display areas. Computerized contents data creating means (18) gives the same identification number to the plural pieces of computerized contents data created on the basis of the outputted information as that given to the pieces of data individually indicating the corresponding display areas (3). In accordance with the identification number, combination means (30) combines the data indicating the plural display areas (3) and the corresponding plural pieces of computerized contents data (6, 7 and 9) individually.
