<P>PROBLEM TO BE SOLVED: To enhance reproducibility of recording information by increasing flatness of an interface between a core layer and a clad layer. <P>SOLUTION: After a UV curing resin is applied onto the core layer 13 to form the clad layer 14, irradiation with UV is performed in such a state that a resin stamper 23 having a rugged pattern corresponding to a rugged part 16 for information is stuck to the surface of the clad layer to cure the clad layer 14. When the resin stamper 23 is peeled after the clad layer 14 is cured, the rugged part 16 for information is transferred to the clad layer 14. After the UV curing resin is applied on the clad layer 14 to form the core layer 13, a smooth base body 22 having a smooth surface such as a resin film is stuck to the surface of the core layer 13 and irradiation with UV is performed in this state to cure the core layer 13. When the smooth base body 22 is peeled after the core layer 13 is cured, the surface of the core layer 13 is flattened.
