PROBLEM TO BE SOLVED: To easily prevent wrong transmission to persons excluding specific persons, when an electronic document is transmitted to a plurality of other persons. SOLUTION: A destination of a read paper document is determined in reference to a registered address book (A), and a transmission permission level is set (D). The paper document describing prescribed items is read out in accordance with a fixed form (B1, B2). The meta-information is created from a scanned document to be attached as additional data of the electronic document (B3). The meta-information is compared with the personal information (C) of the other person to whom the data is distributed and the transmission permission level (D) and determined, so that the transmission is performed if everything is OK (S52), and the transmission is forbidden (S54) or the notification (S58) is given if there is any problem, and an operator is notified of the necessity of determination on transmission. The distribution is forbidden when the response is not received within a prescribed time or the fact of no-distribution is input (S62), and the distribution is performed even to destination out of meta-data range (S64), only when the transmission permission setting is input (S63).
