PROBLEM TO BE SOLVED: To satisfy a total reflection condition for optical signals, that are made incident on a mirror surface, without reducing the NA of a condensing lens. SOLUTION: A collimating lens 142 makes optical signals (laser light beams) 13 emitted from a light emitting element 161 be parallel light beams from diverging light beams. A condensing lens 104 condenses the parallel light beams onto a waveguide port 138 located at one end side of an optical waveguide 135. The optical waveguide 135 reflects the optical signals inputted from the waveguide port 138 by a 45°mirror surface 135a and guides the signals along the longitudinal direction of the optical waveguide 135. The optical axis of the condensing lens 104 is shifted by a distance "d" toward the other end side (an "a" side) of the optical waveguide 135 with respect to the optical axis of the collimating lens 142 so that the main light beam position of the parallel light beams outputted from the collimating lens 142 is separated by the distance "d" toward one end side(a "b" side) of the optical waveguide 135 from the optical axis position of the lens 104.
