PROBLEM TO BE SOLVED: To provide an apparatus for forming slot paper for stably carrying a band slot base paper without a cleaning work. SOLUTION: The apparatus 10 is provided with a carrying section 11 for supplying and carrying the rolled slot base paper 60a into the apparatus, a folding/forming section 12 for folding both ends of the slot base paper 60a and forming a fold, a cutting section 13 for cutting the slot base paper 60a folded and formed in the folding/forming section 12 into a predetermined dimension, a final forming section 14 for forming the slot base paper 60a cut in the cutting section 13 into a U shape, and an adhesive removing section disposed upstream of the folding/forming section 12 and removing an adhesive attached to side faces at the both ends of the slot base paper 60a.
