PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of easily performing color correction at a low cost. SOLUTION: The apparatus comprises a measurement chart output means 130 for outputting a measurement chart including patches of a plurality of colors; a photographic image data acquiring means 100 for acquiring photographic image data including a reference chart containing a plurality of patches whose color density are already known, and the measurement chart; a patch color density holding means 108 for holding the color density of each of the patches included in the reference chart so that each patch may be correlated with patch identification information for identifying each of the patches; and a color density calculating means 126 for calculating the color density of each of the patches of the measurement chart on the basis of the color density of each of the patches correlated to each patch identification information and held by the means 108 and the color density of image data of a patch to be specified by the patch identification information among the pickup image data acquired by the means 100.
