PROBLEM TO BE SOLVED: To provide a magnetic recording medium, in which a recording layer is formed by the prescribed rugged pattern, a recording element is formed as a projected part of the rugged pattern and planar recording density is high, crush of a magnetic head is less likely to occur, and reliability is high, and to provide a magnetic recording and reproducing apparatus provided with such a magnetic recording medium. SOLUTION: The magnetic recording medium 12 includes a recording element 24A, formed as the projected part of a recording layer 24 formed on the prescribed rugged pattern on a substrate 22, and a filling element 28 filled in a recessed part 26 between recording elements 24A, where a surface recessed part 30 of a cross section shape, where a part corresponding to a center part of a width direction of the recording element 24A under it, is dented most to the substrate 22 side and the width, increase monotonously in the direction of separating from the substrate 22, is formed at a surface 32 on the recording element 24A.
