PROBLEM TO BE SOLVED: To provide an optical multiplexer/demultiplexer and an optical multiplex/demultiplex module, which enable a lens and a filter to be integrally composed and which facilitate accuracy of the parallelism and distance for the lens and the filter, as well as alignment operation. SOLUTION: A support plate 33 with a lens 39 formed on one face and a support plate 35 with a lens 41, formed on one face are integrated with a filter 34 held in between the other faces. The support plates 33, 35 are formed with sealing sections 38, 40 in a manner of surrounding the lenses 39, 41 that are larger than the lens thickness. The sealing section 38 of the support plate 33 is stuck with a spacer 31 using adhesive, while the sealing section 40 of the support plate 35 is stuck with a spacer 37 in a similar manner. Both end faces of the optical multiplexer/demultiplexer 301 so composed are each connected with fiber arrays 42, 43 that form the optical multiplex/demultiplex module 401.
