A laminated hologram information storage medium (1) has at least two core layers (2), three or more clad layers (3) sandwiching the core layers (2), one or more diffraction grating layers (4) provided at the boundaries between a part of the core layers (2) and the clad layers (3) or in the core layers (2) and storing information data, and one or more recording layers (42) which is provided at the boundaries between the other part of the core layers (2) and the clad layers (3) sandwiching the other part of the core layers (2), or in the core layers (2), or provided through a gap layer and in which information data is recorded as a shape or a refractive index distribution.
