<P>PROBLEM TO BE SOLVED: To reduce erroneous writing and input errors at the time of preparing a service report and to reduce the burden of customer engineer. <P>SOLUTION: This image forming device is provided with an image forming means for printing out image data and a service report preparing part 115 for storing a form of a service report with maintenance work contents recorded therein and writing inputted information in a corresponding item of a form stored in a storing means in accordance with the execution of a maintenance work. The service report report preparing part 115 uses the image forming means to print out the service report with the information written in the prescribed item.
