PROBLEM TO BE SOLVED: To provide an optical scanner that can simplify the configuration and reduce the cost by efficiently arranging a first laser emitting unit and a first lens and a second laser emitting unit and a second lens, and that can easily adjust positions between the first laser emitting unit and the first lens and between the second laser emitting unit and the second lens, and to provide an image forming apparatus equipped with the optical scanner. SOLUTION: In a laser irradiation optical unit 18, a first holder plate 30 holding the first laser emitting unit 25a and having a first insertion hole 47 formed therein, and a second holder plate 31 holding the second laser emitting unit 25b and having a second insertion hole 51 formed therein, and a base plate 32 supporting a first collimate lens 26a and second collimate lens 26b and having a first screw hole 42 and a second screw hole 43 formed therein, are continuously formed by bending.
