The heating parts (the first heating part 51-1 and a second heating part 51-2) are moved to be in contact with the forming mold 12 by the less number of servomotors than the heating parts. The forming mold 12 after heated is pressurized by a pressurizing part 52 to form a forming base material 11 in the forming mold 12. The forming mold 12 is brought into contact with a first cooling part 53-1 and a second cooling part 53-2 to cool the forming mold 12 after pressurized. The cooling parts (the first cooling part 53-1 and the second cooling part 53-2) are moved to be in contact with the forming mold 12 by the less number of servomotors 1-3 than the cooling parts.
