PROBLEM TO BE SOLVED: To provide a form with a postcard and a hiding label pasted together with a gathering machine without requiring special manufacturing equipment. SOLUTION: This form has a fitness of printing and includes a printing base material 11 comprising a specification part, a postcard part and a hiding label part for hiding the entry surface of the postcard part, a transparent tack paper 12 arranging on the rear side of the postcard part and the hiding label part of the printing base material 11 and comprising a transparent paper, a self-adhesive layer and a releasing paper, an adhesive layer 13 for pasting the printing base material 11 and the transparent tack paper 12, a releasing layer 14, which makes the releasing of the printing base material 11 from the transparent tack paper 12 possible at least in the portion of the hiding label part, a perforation 15 for separating the postcard part from the remaining part and a half-cut 16 for separating the hiding label part from the remaining part.
