PROBLEM TO BE SOLVED: To provide an apparatus for forming an irradiation field of a charged particle beam, which is usable in common for each treatment and reducing the time required for production and the waste of material. SOLUTION: This an apparatus for forming an irradiation field of a charged particle beam, which is for irradiating an irradiated object with charged particle beams 1 accelerated by an accelerator, is provided with an enlarged black peak forming device for enlarging the peak shape of a dose curve of a black peak formed by the charged particle beams 1. The enlarged black peak forming device is provided with layer plates 2 of the same shape formed with slit-like holes 5 periodically in one direction of plates and laminated in an irradiation direction of the charged particle beams 1 so that the slit-like holes 5 face the same direction, and a moving mechanism (not shown in the figure) for moving the layer plates independently of each other in the periodically repeated direction of the slit-like holes 5 and adjusting the moving quantity.
