PROBLEM TO BE SOLVED: To provide a method of issuing card for recognizing which store has issued a card for a customer, and for achieving the efficiency or cost reduction of task processing by using an application form commonly available in respective stores. SOLUTION: The method for issuing a card for a customer available in a plurality of stores based on the entry contents of an application form entered by a customer comprises a step for preparing a plurality of application forms where machine-readable serial management numbers are displayed for each store, a step for mechanically reading the management numbers displayed in the application form entered by the customer by a POS terminal equipment installed in the store when issuing the card to the customer and a step for associating the store number set for each store and preliminarily registered in the POS terminal equipment with the management numbers of the application form mechanically read by the POS terminal equipment, and for writing it as a membership number in the storage means of the card to be issued.
