PROBLEM TO BE SOLVED: To provide a composition for treatment of heart failure, myocardial infarction, hepatic impairment, diabetes, cerebrovascular disease or Parkinson's disease. SOLUTION: A medicinal composition comprises an acyl derivative of uridine or cytidine having formula (I) [wherein R1, R2, R3and R4are identical or different, and each is hydrogen or an acyl group of a metabolite. However, at least one of R1, R2, R3and R4is not hydrogen (the acyl group of the metabolite is an acyl group of a carboxylic acid selected from one or a plurality of a group consisting of a glycolic acid, a pyruvic acid, a lactic acid, an enol pyruvate, an amino acid, a 2-22C fatty acid, a lipoic acid, a pantothenic acid, a succinic acid, a fumaric acid, an adipic acid, an acetoacetic acid, a p-aminobenzoic acid, aβ-hydroxybutyric acid, an orotic acid and creatine.)] or its salt permissible as a medicine.
