<P>PROBLEM TO BE SOLVED: To provide a document issuing system which issues a document in a fixed format, including a specified item when the specified item is inputted, and the document issue system being an output method for the issued document capable of being specified. <P>SOLUTION: When a reception portion 11 of a document-issuing server 4 receives output destination specification information, wherein item information to be included in a document and an output method are specified from a user terminal 1 through the Internet 2, a document creating portion 12 creates the document in the fixed form in accordance with the item information. An output portion 13 outputs the document, created by the document creation portion 12 by the output method specified in the output destination specification information. For example, when the output by HTML is specified, an HTML document is created and transmitted to the user terminal 1.
