PROBLEM TO BE SOLVED: To provide a structure of a liquid discharge head which is more highly functional and fit for densification, and to provide a liquid discharge apparatus and an image forming apparatus using the structure. SOLUTION: The liquid discharge head has a plurality of liquid discharge openings (51), a plurality of pressure chambers (52) each communicating with each liquid discharge opening (51), a diaphragm (56) constituting faces of at least a part of the pressure chambers (52), and a plurality of piezoelectric elements (58) set at a face of the opposite side to a face of the pressure chamber side of the diaphragm 56 and with at least discrete electrodes (57) separated by a range corresponding to the pressure chamber (56). A liquid is discharged from the liquid discharge openings (51) by vibrating the plurality of piezoelectric elements (58). Improvement of a discharging efficiency, liquid droplet volume control, discharging direction control and crosstalk prevention are possible by controlling a driving phase, a driving timing, a driving waveform and the like of each piezoelectric element (58).
