PROBLEM TO BE SOLVED: To achieve miniaturization/lightening in weight in a camera module even when incorporating an autofocus function and a zoom function by using a piezoelectric element. SOLUTION: A first lead screw 18 and a second lead screw are respectively engaged with first and second lens holders, and a first rotor 20 and a second rotor are respectively attached to the first and the second lead screws. A first piezoelectric element 26 and a second piezoelectric element are respectively held in a first holding part (piezoelectric element supporting part) 25 and a second holding part, and the first and the second holding parts are respectively arranged nearly in parallel with and nearly proximately to the first and the second lead screws. The actuation part 26a of the first piezoelectric element and the actuation part of the second piezoelectric element are respectively brought into press-contact with the upper surfaces of the first and the second rotors, and the first and the second piezoelectric elements are excited to drive and rotate the first and the second rotors, whereby the first and the second lens holders are moved along an optical axis.
