PROBLEM TO BE SOLVED: To obtain a medicine for bringing about an effect excellent in not only acute lung injury and DIC (disseminated intravascular coagulation) involving septicemia, but also in prophylaxis and/or treatment of multiorgan failure in liver, kidney, and the like and improvement of the prognosis of a patient by a combination of an elastase inhibitor with an enzyme inhibitor of blood coagulation system and/or fibrinolysis system. SOLUTION: The present invention relates to the medicine comprising a combination of the elastase inhibitor comprising, for example, a compound represented by formula (IA) or a nontoxic salt thereof with the enzyme inhibitor of the blood coagulation system and/or fibrinolysis system comprising, for example, compounds represented by formulae (IIB), (IIC), (IID) and (IIE), an aprotinin derivative, an antithrombin III derivative, a heparin derivative, a danaparoid sodium derivative, a hirudin derivative or a thrombomodulin derivative or a nontoxic salt thereof.
