PROBLEM TO BE SOLVED: To provide surgical radiological apparatus and a positioning method using the apparatus allowing a precise surgical procedure even if changing a display state. SOLUTION: When the display state of a monitor 37 is changed in a case where superimposition marks (display indices) are superimposedly displayed at a position on an X-ray fluoroscopic image corresponding to a laser (an external index) indicating a position to be treated by the surgical procedure on a subject, an index following part 34a makes the display of the mark follow the position of the X-ray fluoroscopic image corresponding to the laser based on change information (a position relationship and image processing information) so that the surgical procedure can be performed based on the laser with the mark following the laser.
