PROBLEM TO BE SOLVED: To provide a surgical instrument easily penetrating hard tissue such as muscles to perform surgery without damaging internal organs, muscles or the like in surgery for treating abdominal pressure urinary incontinence by leading a urethra supporting tape into a woman's abdomen. SOLUTION: The surgical instrument having a bar member 2 at least partially curved, and the support tape 1 attached to the proximal end of the bar member 2, leads the support tape 1 into the woman's abdomen using the bar member 2 and supports the urethra to the support tape 1 to treat the abdominal pressure urinary incontinence, wherein the bar member 2 is constituted to have at least two forms of improving a dilacerating function of the tip part when penetrating tissue and lowering the dilacerating function of the tip part when passing through a body cavity.
