PROBLEM TO BE SOLVED: To provide a document reader and an image forming apparatus which are capable of reducing stripe noise caused by foreign matters on a read glass by not oscillating the read glass with a high frequency when reading a color image. SOLUTION: An image read part 102 constituting an MFP reciprocates a read glass holder 211 in a subscanning direction with a low period of 18 times per second by using a crank mechanism comprising a motor 217 and gears 215 and 216. A movement stroke of the read glass 203 is 9 mm. Thus a length of a stripe part of stripe noise occurring with a foreign matter having a diameter of 100μm stuck to the read glass 203 is 2.57 mm, and intervals of stripes are 7.27 mm, so that the stripe noise is made inconspicuous.
