PROBLEM TO BE SOLVED: To provide an apparatus for forming an optical device and a method of manufacturing the optical device, wherein the optical device having a desired characteristic is stably obtained and the damage or the shortened life of the optical device-forming apparatus is avoided. SOLUTION: The apparatus for forming the optical device is provided with a heater for heating a base material to be formed, a pipe line for circulating cooling water in the apparatus and a thermometer for detecting the temperature of the circulating cooling water. When the temperature of the cooling water measured by the thermometer exceeds the set value, the heater is controlled to lower the temperature of the heater. The optical device having the desired optical characteristic is stably and safely formed with high productivity by monitoring the forming condition regularly and lowering the temperature of the heater when an abnormal state is detected.
