<P>PROBLEM TO BE SOLVED: To form an image on the label surface of an optical disk by utilizing a laser beam outputted from an optical disk device. <P>SOLUTION: A visible light characteristic changing layer formed of a photosensitive material, a heat-sensitive material or the like is formed in a place which can be viewed from a part of the label surface of the optical disk. The optical disk is set to the turntable of the optical disk device while the label surface is directed downward. The optical disk and an optical pickup are relatively moved along the surface of the optical disk. At this time, a tracking servo is turned off and a focus servo is turned on or off. The power of the laser beam emitted from the optical pickup synchronizing with the relative movement is modulated in accordance with image data of a character, a picture and the like to be image-formed and the visible light characteristic changing layer is irradiated with the laser beam.
